The Surrogacy Institute of Hawaii (SIH) is the Fertility Institute of Hawaii’s full-service surrogacy/ Gestational Carrier agency. Intended parents are welcome to utilize the services of SIH or an outside agency should they require a Gestational Carrier (GC) or surrogate.

What is Surrogacy?

Surrogacy involves a woman (the gestational carrier or surrogate) carrying a pregnancy for another individual or couple (the intended parents) using an embryo created through in vitro fertilization (IVF). The purpose of surrogacy is to help individuals or couples who are unable to conceive or carry a pregnancy to term due to various medical, biological, or personal reasons.

What services are offered by the Surrogacy Institute of Hawaii?

Personalized Matching and Screening Services: SIH recruits, screens, and educates potential surrogates. Candidates undergo a comprehensive evaluation, including a medical history review, psychiatric screening, and physical exam. Intended parents are then allowed to review appropriate pre-screened surrogates. If all parties agree, intended parents and surrogates are allowed to meet in person or virtually before committing to the process.

Education

SIH provides comprehensive, step-by-step instruction and guidance regarding the complex surrogacy process.

Legal Support

Legal protection for both the surrogate and the intended parents is essential to ensure legal parentage and that all parties provide informed consent. SIH facilitates legal services and verifies that appropriate legal contracts are in place.

Medical care coordination

SIH coordinates IVF services and the prenatal care of the surrogate, from preconception counseling to successful delivery, to ensure that both the surrogate and intended parents receive appropriate care and communication between all parties throughout the process

Emotional Support

SIH provides emotional and educational support to all parties involved in navigating any challenges that may arise during the surrogacy journey. Referral to mental health counseling can be provided if needed.

What is the process like for surrogates and intended parents?

STEP 1: Initial Consultation:

If not already established, intended parents can establish care with the Fertility Institute of Hawaii to discuss their personalized treatment plan. Potential surrogates will also have an initial consultation to answer questions and evaluate suitability for the process.

STEP 2: Application and Screening

Surrogates undergo comprehensive screening, which includes the following, and is coordinated by SIH

STEP 3: Matching

Intended parents and suitable surrogates will be connected with each other and allowed to decide whether they wish to proceed with the process.

STEP 4: Legal Agreement

Surrogates and intended parents consult with attorneys (facilitated by SIH) and complete a legal contract.

STEP 5: Medical Procedures

Intended parents create embryos using the eggs and sperm of intended parents or appropriate donors. Surrogates undergo frozen embryo transfer.

STEP 6: Prenatal care

Surrogates undergo the same type of prenatal care as any other pregnant woman who has undergone IVF. A physician at the Fertility Institute of Hawaii will follow the surrogate through 8 weeks of pregnancy; then, the surrogate will continue regular obstetric care with their OBGYN for the remainder of the pregnancy.

STEP 7: Delivery and postpartum care

Depending on the state and legal contract, post-birth paperwork may be required to establish parentage. Routine delivery and postpartum care are provided by obstetricians and coordinated by SIH.

What are the qualifications for being a surrogate?

Both surrogates and intended parents must meet specific criteria before entering into a surrogacy arrangement to ensure the safety, health, and success of the pregnancy.

Criteria for Surrogates:

      1. Age: Surrogates should be between the ages of 21 and 45.
      2. BMI: should be between 18 and 36 kg/m2
      3. Health: Surrogates should be in good physical and mental health, with no major medical conditions that could affect pregnancy, and without drug use or current sexually transmitted infections.
      4. Previous Pregnancy: Surrogates must have had at least one successful, full-term pregnancy. The following pregnancy and delivery history may be evaluated on a case-by-case basis:
        • OB records must be available for MFM review.
        • Should have at least one term, uncomplicated pregnancy.
        • No history of pregnancy complications or preterm deliveries.
        • No more than 5 prior deliveries.
        • No more than 3 prior c-sections
        • A history of no more than 1 blighted ovum or miscarriage
        • Must wait for a minimum of 1 year following a vaginal delivery or following c-section
        • Must be finished breastfeeding
      5. No History of Substance Abuse: A history of substance abuse or mental health disorders may disqualify a woman from becoming a surrogate.
      6. Stable Lifestyle: A surrogate should have a stable family and home environment.
      7. Non-Smoker: Must be a non-smoker or avoid tobacco use during the pregnancy.
      8. Psychological Screening: Surrogates must undergo a psychological evaluation to assess emotional preparedness and motivation.
      9. US Citizen: Must be a US citizen or able to legally work in the US.

What costs are involved in the surrogacy process, and how are surrogates compensated?

The average cost of surrogacy in the US can range from $100,000 to $200,000 and is dependent on various factors. These costs are typically made up of the following:

Fertility Treatment Expenses – $10,000 – $50,000+

Intended parents will need to undergo In Vitro Fertilization to create embryos, which will be used for transfer to the surrogate. These treatments can vary widely based on your medical diagnosis.

Agency Fee – $30,000 – $40,000

This fee covers recruitment, matching, and coordination of all medical care throughout the surrogacy journey as well as emotional and legal support. Please note that the exact fee may vary depending on where the surrogate and intended parent(s) reside due to additional coordination and legal services required when either party is located out of state.

Surrogate Base Compensation – $60,000+

Surrogates are compensated for their time, effort, and medical risks with an amount that the surrogate and intended parent(s) agree upon

Surrogate Additional/Contingent Compensation – $20,000+/-

Additional fees intended parents can expect to compensate their surrogate for will depend on whether certain contingencies occur, which may include, but are not limited to, the following:

  • Insurance coverage for the surrogate
  • Invasive procedures
  • Additional screening or evaluation of the surrogate
  • Travel expenses (as needed)
  • Lost wages

Legal Fees – $5,000 – $10,000+/-

The intended parents are responsible for the legal fees for their own and the surrogate’s legal counsel.
